I think the only way that this could be succesfully done would be by amputation at the ankle followed by the fitting of a rigid vertical titanium prosthetic foot with a fixed 20 cm titanium heel attached. Like a hip joint, the foot would be socketted into the ankle bone with very limited flexibiity. Once the pain subsides you will have to learn to walk all over again. Fully enclosed and tightly laced rigid ankle boots will be essential at all times as your ankle will be very vulnerable to fracture. Your boots will have to be speciallly made with an opening to push your permanent heels through. Your legs will look fantasticaly long and slender and if you wished to go for a 25 cm heel, it and the toe piece could be unscrewed from the foot and replaced. Obviously this is a lifetime commitment to extreme heels. Wow, I hope that you succeed.
Just start wearing high shoes 24/7. You will re-mould your feet to the shape of the high heels.
Just start wearing the heels short periods of time. Then start extending these short periods into longer ones.
